we are

MATCH addresses a critical need in McDowell County by increasing access to whole-person, coordinated care for our most vulnerable neighbors. Recognizing that access to healthcare alone isn’t enough to ensure that individuals thrive, MATCH offers a uniquely designed model that goes beyond traditional healthcare. Through personalized support and resource navigation, the program connects participants to vital services—such as food, housing, transportation, and more—while providing ongoing support as they work toward their own self-defined goals.

Mission:

To reduce health disparities for McDowell residents by facilitating access to the services people need to get well, be well, and stay well.

MATCH envisions a McDowell County where all people have equitable access to care and resources that they need to be healthy.

Any McDowell County resident in need of access to health care services, local and regional medical providers, and community agencies.

MATCH is a partnership between CareReach and Mission Hospital-McDowell. MATCH relies on strong collaborations with community agencies to help ensure McDowell residents have access to the resources needed to thrive.
